Thank you for your interest in becoming a Vocational Rehabilitation (VR) provider for Opportunities for Ohioans with Disabilities (OOD). OOD partners with private and public providers to assist individuals with disabilities to obtain and maintain competitive integrated employment within the community.
If you are interested in being added as a medical (physical therapist, hearing aid provider, etc.), psychological, or dental supplier of services, please contact a local office and ask to speak to an Account Examiner. The Account Examiner will be able to discuss standard rates, billing processes, and working together to support individuals with disabilities.

Step 2: Determine what services you intend to provide by reading the VR Provider Manual at https://ood.ohio.gov/wps/portal/gov/ood/information-for-providers/provider-resources/provider-manual and reviewing Qualifications and standards for service providers and vendors at http://codes.ohio.gov/oac/3304-2-53v1
Step 3: Once you have all materials and credentials ready, create an application in the Provider Management Program (PMP) at https://oodpacprd.powerappsportals.us/ . Submit all documentation through the PMP including your most recent credentials and Form W-9.
Step 4: Register on the statewide Supplier Portal at http://supplier.ohio.gov and sign up for OOD provider communication emails near the bottom of the OOD webpage at http://ood.ohio.gov, entering your email in the “Subscribe to OOD Weekly Updates” and selecting the “Vocational Rehabilitation (VR) Providers” checkbox on the next screen.
Step 5: Attend New Provider Orientation with a Rehabilitation Program Specialist (RPS) in the Provider and Contract Management Unit (PCMU).
